Getting started is simple.
Patients may self schedule their first one hour initial appointment directly on this page. This visit is designed to establish care and complete a thorough evaluation.
You do not need to start a membership to book your first appointment.
After selecting a date and time, you will be prompted to complete basic information. Full demographic forms can be completed afterward for patients age 18 and older, or by a parent or guardian.
Patient intake forms, payment, and confirmation details will be provided at the end of scheduling.
Telehealth Appointments
All appointments are conducted via HIPAA compliant telehealth platforms, both during your session and for any follow up communication.

First Time Here?
Looking to schedule a first-time appointment? No problem. You can book your initial one-hour visit, whether you are planning to start a membership or prefer to pay per visit.
If you are referred by a healthcare provider or a friend, your first visit is 50% off.
You will have the option to indicate referral information during scheduling.
